David Free
Executive Director

David Free, DNP, MS, NP-C, FNP-BC, ACHPN  completed his undergraduate degree in Nursing at McMaster University,  a Master’s degree at University of Maryland Baltimore as a Primary Care Nurse Practitioner, and a clinical Doctorate in Nursing at Chatham University. He is also an adjunct professor at Brock University School of Nursing.

Dr. Free has published several book chapters on hospice palliative care in nursing and medical textbooks as well as a number of articles in peer-reviewed journals.  David has presented his work and has been invited to speak at national and international conferences.  He completed a one-year fellowship in Palliative Care and Pain Medicine in New York City, and is nationally certified in palliative care in Canada and the U.S.   David been engaged in hospice palliative care clinical practice, program development, and systems leadership in the U.S. and Canada for 18 years.

Kim has worked and volunteered in the not for profit sector for more than 18 years. She has held roles in all areas of the industry including annual programs, event management, planned giving, major gifts and campaign management, in both local and national organizations, primarily in the health care field.
Kim has a keen interest in advancing the field of fund development, particularly in health care. She obtained her CFRE in 2015. She has been an active member of the Association of Fundraising Professionals and Association of Healthcare Professionals. Since 2016 she has been teaching AFP Essentials of Fundraising, the CFRE Review Course, and has presented at several conferences. She is currently pursuing her Masters of Philanthropy and Nonprofit Leadership.

Jo Ann has over 30 years of administration experience. Jo Ann experienced the amazing care and compassion provided by the staff and volunteers during her father-in-law’s final weeks at Lisaard House prior to joining our team. Jo Ann has been with Lisaard House since 2012. Jo Ann has completed the Volunteer Management program at Conestoga College and is responsible for our volunteers and operations. One of her favourite quotes sums up her views on volunteering and teamwork: “I can do what you cannot do and you can do what I cannot do. Together, we can do great things.” – Mother Theresa

Kate Wiens, RN, BScN, CHPCN : Resident Care Manager

Clinical Resource Manager

As a Registered Nurse with CHPCN certification in Hospice Palliative Care, Kate began with Lisaard and Innisfree Hospice as a BScN student.  After specializing in inpatient oncology, she joined our team as a frontline RN in 2010. Kate is now the Clinical Resource Manager, overseeing the resource team and staff education and training.  Kate has dedicated her career to the provision of exceptional end of life care for residents of the community and their families. Kate also holds diplomas in Social Services and Social Work as well as a Bachelor of Arts majoring in Social Development Studies and minoring in Sexuality, Marriage and the Family.

Social Worker

Heather MacCuaig is a Registered Social Worker having worked in the Oncology and Palliative Care Program at Cambridge Hospital. Heather has taught in the Palliative Care Certificate Program at Conestoga College assisting in the development of the Bereavement Issues course and is currently an Assistant Clinical Professor at the School of Medicine at McMaster University.

Greg Cressman : Bereavement Coordinator

Greg is a registered nurse whose main focus for over 20 years has been the provision of palliative care. He has worked as a Visiting Nurse, and as a Clinical Nurse Specialist at St. Joseph’s Hospital in Hamilton. Greg has a Master’s degree for which he published narrative research on Alternate Level of Care. In 2015, he and his family moved from Hamilton and he is pleased to serve this community in his new role.

Debbie Martins : Operations Assistant

Debbie has been with Lisaard and Innisfree Hospice since 2009 as a Personal Support Worker. Debbie has 10 years of palliative care experience. She has now transitioned into an administrative role as our scheduler/office assistant where she brings her experience and passion to providing effective staff coverage for our residents.

Clinical Resource Nurse Team

This team is made up of experienced palliative nurses who demonstrate clinical expertise, knowledge and professionalism to help coach and mentor our clinical staff. Under the direction of the Clinical Resource Manager, the resource nurse team facilitate resident intake, placement, and works to ensure a smooth transition of residents to the hospice. This team facilitates strong working relationships between our residents, families, inter-professional team members and community partners.
